Request for Make Up Hours

This form is intended for students who need to schedule attendance outside of their normal schedule because they have missed class. Your instructor or an administrator will reach out to you if you need to schedule make up hours.

  • Please indicate whether you missed clinic floor appointments or classroom theory. If you missed class, please indicate the material covered in your absence.
  • Make-up hours are available on the dates shown below. Requests will be closed one week prior to the make-up dates. This means if you would like to request make-up hours for October 22, your must make the request by no later than October 15, and etc.
  • Make-up hours are allotted in 2-hour blocks, unless you have prior approval to select the 5-hour option.
  • Because time for make-up hours is limited, priority will be given to students who missed school because of an unavoidable exigency (i.e., illness, injury, court order, etc.). Please upload your doctor’s note or other documentation.

    • Important Reminders

      Make-up hours are granted on a first-come, first-served basis. Requests must be for a minimum of two hours. Available dates appear above. Because we have limited clinic availability, requests for make-up hours must be approved. This request is not an approval. If you miss approved make-up hours, you MUST provide documentation from a medical doctor, court, DES, etc. or you will be prohibited from requesting future make-up hours.
